Why the Whole Cyst Has to Come Out
A sebaceous or epidermoid cyst is a sac lined with skin cells that continues producing keratin. Drain the contents and leave the sac, and it refills. That is why squeezing achieves nothing lasting, and why an incision and drainage at an out-of-hours appointment often ends with the cyst returning weeks later.
Proper removal takes out the entire sac wall. Done electively, on skin that is not inflamed, it is a small procedure with a neat scar.
Timing matters. If a cyst becomes infected, the tissue planes are distorted and the sac is far harder to remove intact — so the infection is usually settled first and the cyst removed some weeks later. If yours is currently red, hot and painful, come and be seen, but expect a two-stage plan.
When to Have One Removed
Many cysts need nothing. Consider removal if yours is enlarging, has become infected once or more, catches on clothing or a razor, is cosmetically bothersome, is in an area under pressure such as the back or waistband, or if you are simply not certain what it is.
That last point matters. Not everything that looks like a cyst is one. Lipomas, pilar cysts, abscesses and, rarely, malignant lesions can present similarly. We examine before we remove, and send specimens for histology where indicated.
