What Is Hyperhidrosis?
Hyperhidrosis is sweating beyond what the body needs for temperature regulation. It affects roughly one to three people in a hundred. It is not caused by anxiety — though it reliably causes it — and it is very treatable.
Sweat glands are controlled by the sympathetic nervous system. In hyperhidrosis the signal to those glands is excessive: the glands themselves are normal, the instruction is not.
Primary focal hyperhidrosis is the common form. It affects specific areas — usually the underarms, palms, soles or face — it is symmetrical, it typically starts in adolescence, it often runs in families, and it stops during sleep.
Secondary hyperhidrosis is generalised, can occur at night, and is caused by something else: thyroid disease, diabetes, menopause, infection, lymphoma, or medication. Night sweats and generalised sweating need investigating before treating, and we will arrange it.
The distinction matters. Treating secondary hyperhidrosis cosmetically without finding the cause is bad medicine.
How Does Treatment Work?
Sweat glands are activated by acetylcholine — the same neurotransmitter that activates muscle. Botulinum toxin blocks its release at the sweat gland just as it does at the neuromuscular junction. A grid of small, shallow injections across the affected area switches off the signal.
Sweating reduces within three to seven days and is typically cut by 80–90%. The effect lasts six to nine months — longer than cosmetic anti-wrinkle treatment, because sweat gland nerve endings regenerate more slowly.
Compensatory sweating elsewhere is uncommon with injection treatment, unlike surgical sympathectomy.

Underarms — £450. The most commonly treated area and the most reliably successful. Roughly 20 injection points per side, ten minutes, minimal discomfort.
Forehead and scalp — £350. For facial sweating that ruins makeup, fogs glasses and is impossible to hide. Requires care to avoid affecting the brow, which is one reason this is a doctor-administered treatment at CLNQ.
Palms and soles. Effective but more uncomfortable to treat, because the skin is thick and richly innervated. We discuss nerve block options at consultation.

Who Is Suitable For Treatment?
You are likely to benefit if sweating soaks through clothing, forces you to change during the day, dictates what colours you wear, interferes with work or handwriting, or has not responded to prescription-strength antiperspirant containing prescription-strength antiperspirant.
We will normally ask you to have tried a prescription antiperspirant first, because for milder cases it works and costs very little.
It is not suitable during pregnancy or breastfeeding, with a neuromuscular disorder such as myasthenia gravis, with an infection at the site, or where a secondary cause has not yet been investigated.
What Else Can Be Done?
- Prescription antiperspirants — prescription-strength antiperspirant, applied at night. First line, and genuinely effective for mild to moderate cases.
- Iontophoresis — particularly for palms and soles.
- Oral anticholinergics — effective but with systemic side effects, so reserved.
- Microwave and radiofrequency ablation — permanently destroys underarm sweat glands.
- Surgical sympathectomy — a last resort, with a meaningful rate of compensatory sweating elsewhere.
Most patients do best with injections, and our job is to tell you when you would not.
What Are The Risks?
Injection site soreness, small bruises and brief stinging during treatment. Occasionally mild compensatory sweating in an untreated area. In facial treatment, temporary brow asymmetry if the toxin spreads — avoidable with correct depth and placement. Temporary hand weakness can follow palm treatment, which is why we discuss it carefully before treating hands.

What areas can Botox be used for to help sweating?
The most common area for hyperhidrosis treatment with Botox is the armpit (axilla). The other areas where it can be used include the forehead, palms and soles of feet.
Does it completely stop sweating?
Botox for hyperhidrosis does not completely stop sweating but it will reduce it. Studies have shown that most patients will notice from 75-90% reduction in sweating. The research papers show that this treatment has a very positive impact on quality of life after treatment.
How long does the hyperhidrosis treatment last?
The botulinum toxin treatment for hyperhidrosis is a temporary treatment and generally lasts up to about 3-4 months. In some it may last less and in some longer. Individual response is variable.
How does botulinum toxin work for hyperhidrosis?
Botulinum toxin, commonly known as Botox, block the nerves supplying the ecrrine glands responsible for sweating. It can be used to treat localised areas of excessive sweating such as the armpits, palms or soles of feet.
