What actually clears blackheads, and what does not

Blackheads are trapped sebum and dead skin that has oxidised and darkened at the surface. Nothing that only cleans the surface will empty a blocked follicle, which is why so much of the home routine people arrive with has made no difference.

Blackheads: common questions

Are blackheads caused by dirt?

No. The dark colour is not dirt and it is not a sign that your face is unwashed. A blackhead is a follicle plugged with sebum and dead skin cells; the surface of that plug darkens when it is exposed to air, in the same way a cut apple browns. Washing more often, or more aggressively, does not clear them and usually irritates the skin into producing more oil.

What is the difference between blackheads and sebaceous filaments?

This catches a lot of people out. Sebaceous filaments are the normal grey or tan dots you see on the nose and inner cheeks — they are the natural lining of the follicle doing its job of moving oil to the surface, and everybody has them. Blackheads are genuine blockages, darker, and sit slightly raised. Filaments cannot be permanently removed because they are supposed to be there; trying to squeeze them out just stretches the pore. If your “blackheads” cover the whole nose evenly and refill within days, they are almost certainly filaments.

Should I squeeze blackheads at home?

We would rather you did not. Squeezing with fingernails ruptures the follicle wall below the surface, which spreads the contents into the surrounding skin and produces the red, inflamed lump that then takes weeks to settle and can leave a mark. Most of the acne scarring we treat started as home squeezing. If a blackhead genuinely needs to come out, extraction after the skin has been softened, using sterile technique, does far less damage.

Do pore strips work?

They remove the top of the plug and give a satisfying result on the strip itself, but they leave the rest of the blockage behind and the pore refills within days. Used often they irritate the skin and can worsen redness. They are not harmful occasionally; they are simply not a treatment.

How do I stop blackheads coming back?

By changing what happens inside the follicle rather than only cleaning the outside. That usually means a leave-on exfoliant containing salicylic acid, a retinoid where your skin tolerates one, non-comedogenic products, and a professional treatment such as HydraFacial or a peel at intervals.
