How Do Vitamin B12 Injections Work?

B12 taken by mouth has to survive stomach acid, bind to a protein called intrinsic factor, and be absorbed in the last section of the small intestine. Several common things interrupt that chain.

  • A vegan or vegetarian diet provides very little B12 to begin with
  • Ageing reduces stomach acid production
  • certain diabetes medication and long-term acid-reducing medication reduce absorption
  • Bariatric or bowel surgery can remove the very part of the gut that does the absorbing

An intramuscular injection skips all of it. The B12 goes into the deltoid muscle and into circulation directly, so absorption is not in question. Levels rise within hours rather than weeks.

CLNQ uses the long-acting form of B12, the form used by the NHS, which stays in the body longer than the shorter-acting synthetic form and therefore needs fewer repeat doses.

What Are The Benefits Of Vitamin B12 Injections?

Being honest about this matters, because a great deal of what is written about B12 online overstates it.

Where the evidence is strong — if you are deficient:

  • Energy and fatigue. B12 is essential to red blood cell production. Deficiency causes megaloblastic anaemia, and the tiredness that comes with it is profound. Correcting the deficiency resolves it.
  • Nerve function. B12 maintains the myelin sheath around nerves. Deficiency causes pins and needles, numbness and, if prolonged, permanent nerve damage. This is the reason not to leave a deficiency untreated.
  • Cognitive clarity. Brain fog, poor concentration and memory difficulty are recognised features of deficiency and improve as levels are restored.
  • Mood. B12 is a cofactor in the synthesis of serotonin and dopamine.
  • Red blood cells, DNA synthesis and homocysteine metabolism all depend on adequate B12.

Where the claims outrun the evidence. If your B12 level is already normal, an injection is unlikely to give you more energy than you have now. There is no good evidence that supra-normal B12 levels improve performance, accelerate weight loss or slow ageing in people who are already replete. We would rather tell you that before you book than after.

Who Is Suitable For B12 Injections?

B12 injections are worth considering if you:

  • follow a vegan or vegetarian diet
  • are over 50, when stomach acid production falls
  • take certain diabetes medication, or long-term long-term acid-reducing medication
  • have had gastric bypass, sleeve gastrectomy or bowel resection
  • have pernicious anaemia, coeliac disease or Crohn’s disease
  • have tested low or borderline on a B12 blood test
  • experience persistent fatigue, pins and needles, brain fog or mouth ulcers that have not been explained

They are not suitable if you have a known cobalt or cobalamin allergy, or Leber’s hereditary optic neuropathy. If you are pregnant or breastfeeding we will discuss it with you individually.

How often should you have B12 injections? For a diagnosed deficiency, the usual pattern is a loading course — an injection every few days for the first two weeks — followed by maintenance every eight to twelve weeks. For maintenance in people with dietary or absorption risk rather than frank deficiency, every eight to twelve weeks is typical. We set the interval from your blood results and your symptoms, and we re-test rather than guessing.

What Are The Risks?

B12 is water-soluble, so excess is excreted rather than stored, and the safety profile is good. Reported effects are mostly local and short-lived:

  • soreness, redness or mild swelling at the injection site
  • brief lightheadedness
  • mild nausea or headache
  • rarely, acne or a rash
  • very rarely, an allergic reaction

Injection carries a small risk of bruising and, rarely, local infection. Having it done by a clinician in a clinic rather than at a kitchen table meaningfully reduces that.

If you have an undiagnosed anaemia, treating it with B12 alone can mask a folate deficiency. This is a real reason to have bloods checked first rather than an upsell.

Do I need a blood test first?
We strongly recommend one. It tells us whether you are deficient, how deficient, and whether something else is going on. Blood testing is available at CLNQ and can be arranged at the same visit.

How quickly will I feel a difference?
If you are genuinely deficient, most people notice a change in energy within a few days to two weeks. If your levels were already normal, you may notice nothing — and that is the correct outcome, not a failed treatment.

Does it hurt?
It is a small intramuscular injection into the upper arm and takes seconds. Most people describe a brief sting and nothing more.

Can I have B12 injections if I am vegan?
Yes, and vegans are among the people most likely to benefit. the long-acting form of B12 is produced by bacterial fermentation and is not animal-derived.

Can I just take tablets instead?
If your absorption is intact, high-dose oral B12 can work. If it is not — which is the case for most people with pernicious anaemia, after bariatric surgery, or on long-term certain diabetes medication or PPIs — tablets will not solve the problem regardless of dose.
