Are You Actually A Candidate?
We are putting this first, because the most useful thing this page can do is stop some people booking.
Under-eye shadowing has four different causes, and only one of them responds well to filler.
1. Volume loss and a true tear trough hollow. Filler works well. This is the indication.
2. Herniated fat pads — under-eye bags. The bulge is orbital fat pushing forward, and the shadow is the crease below it. Filling that crease can camouflage a mild case; in a moderate or significant one it makes the whole area heavier. The right treatment is lower blepharoplasty, and being able to tell you that — and to perform it — is the advantage of being assessed in a plastic surgery clinic.
3. Pigmentation. Genuine melanin darkening, common in darker skin types and often familial. Filler does nothing for it. Skin brightening, careful peels or laser are the options.
4. Thin, crepey skin showing the vasculature beneath. Adding volume is not the answer; improving skin quality is. Polynucleotide treatment at £300 is the better choice, and it is what we would recommend.
Most people are some combination. We will tell you which you are, and if the answer is “not filler”, we will say so.
How Is Tear Trough Filler Done?
After assessment and topical anaesthetic, a soft, low-hydrophilic hyaluronic acid filler is placed deep — on the bone, beneath the muscle — using a blunt-tipped cannula through a single entry point on each side.
Depth and product choice are what separate a good result from a visibly bad one. Filler placed superficially here produces the Tyndall effect: a blue-grey cast that is unmistakable once you know to look for it.
Small volumes. Usually 0.5–1ml across both sides in total. Under-correcting and reviewing at two weeks is the right approach — the area is unforgiving of enthusiasm.

Every treatment is quoted in writing before you commit. Results typically last nine to eighteen months, and some patients retain product in this area considerably longer.
What To Expect Afterwards
Swelling for three to five days — and the under-eye area swells more visibly than anywhere else on the face. Bruising is common and can last a week, because the skin is thin and the vessels are numerous.
The result settles at two to four weeks. Do not judge it before then.
Sleep propped up for two nights, avoid salt and alcohol for a few days, and do not book this a fortnight before a wedding.
What Are The Risks?
- swelling and bruising — common and expected here
- prolonged puffiness — hyaluronic acid attracts water and the under-eye tissue holds it; the most common reason for dissatisfaction
- Tyndall effect — bluish discolouration from superficial placement
- lumps, irregularity or asymmetry
- vascular occlusion — the infraorbital and angular vessels run here, and this region carries one of the highest risks of blindness from filler anywhere on the face
Vascular occlusion is rare. It is also the single strongest reason to have this treatment done by someone who injects with a cannula, knows the anatomy, recognises occlusion instantly and has hyaluronidase in the room. We keep it on site.
All of it is reversible with hyaluronidase (£150), which is a genuine advantage of hyaluronic acid over anything permanent.

Will it get rid of my dark circles?
If the darkness is a shadow cast by a hollow, yes. If it is pigmentation or visible vasculature through thin skin, no — and we will tell you that at consultation rather than after you have paid.
Can it be dissolved?
Yes, completely, with hyaluronidase.
How long will I be bruised?
Up to a week is normal here. Avoiding blood-thinning supplements beforehand helps.
Filler or polynucleotides?
Filler replaces volume. Polynucleotides improve skin quality. Hollowing needs filler; thin crepey skin needs polynucleotides; many people benefit from both, sequenced.
Why is it more expensive than other filler areas?
Because it has the least margin for error of anywhere on the face, it takes longer, and it is assessed and injected by a doctor who can also tell you when the answer is surgery rather than filler.
